for malaria, bali is a low risk zone, but lombok is higher so you will definitely need anti-malarial for there, but double check with a travel nurse. the two drug options i was offered were malarone (more expensive, fewer side effects) and doxycycline (less expensive more possible side effects and have to take it for longer).

For cost, malarone is usually double the price, so I went for the latter. Shop around on the internet for cheap prices (have to have prescription). If you're in the Uk travelpharm.com are pretty good for price.
protection it two-fold, anti-malarial (see above) and anti-mosquito (i.e. minimise bites using nets, sprays, long sleeve clothes, DEET etc)

other vaccinations: you may already have some of these or just need a booster:
Tetanus/ Diptheria/ Polio
Rabies (expensive and only gives few extra days before having to go to hospital, I left it too late to get a jab, so I'm just going to avoid monkeys, dogs etc)
Hepatitis B (good idea to get that, but again need time to get the full course, so go see your travel nurse asap)
If you're coming from a yellow fever zone country you'll need that jab and a certficate as well.
