Boards-board shops around poppies 2 in kuta, dont know if can rent, expect soo, or you could buy then sell back at end of trip? possibly better if your there for a while or go to other islands.
Regarding places to visit or stay, check out lonely planet website or do search on net, to much to write here, same goes for accomadation, thousands of options there are review sites for bali accomadation or there are cheap options near waves on bukit, like bingin and ullus.
Boat trips?...If thinking of doing a boat trip, to lombok, sumbawa, check out my website all listed there with links, follow my e-mail addy.
Nusa lembongan is nice side trip for a few days if its your first Bali/Indo trip as is lombok or Lakey peak.

For the boards, try rentasurfboard.co, they have a website plus a shop in jalan Benesari. (Kuta) Better not rent from the beach.
For the boatd trips, in Poppies 2 and Benesari, you hqve plenty of tour operators that organize boat trips, you can't miss them.
Once you get your board, your tour, run away fro Kuta, definitly not the best place and the best surf. go South to Bukit if you want reef bootom, northto canggu if you want sand.
