As long as I can see at this website (http://www.bcm-surfpatrol.com/), it looks like there are only two major surf spots in Hokkaido: Hamaatsuma near Tomakomai, and Itankihama near Muroran. I heard from a friend that there's another spot near Hakodate where you can surf when a typhoon comes, but expect that that spot rarely gets surfable swells and waves, when any typhoon is not there.
