I've never surfed Miramar but have seen video from when my buddy surfed there. Good left barrel spot that breaks at high tide. No one else out. They call it the pipes. Hope to go this trip.

there is a hotel in puerto sandino 5 minutes by car, the hotel provide a guide or boat trips
